Authorize to exercise a renewal option to an existing agreement with the Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 9-27-17, item #13), for the provision of assistance and support for 13 NH small rural hospitals in order to implement activities provided annually by the Small Rural Hospital Improvement Program Grant, by increasing the price by $281,197 from $212,494 to $493,691, and by extending the completion date from May 31, 2019 to May 31, 2021. Effective upon G&C approval. 100% Federal Funds.
