Authorize the Office of Quality and Improvement to enter into an agreement with Westport Healthcare Management Inc., d/b/a Pacific Health Policy Group, Highland Park, IL, for the provision of the development of evaluation plans for the Substance Use Disorder, Section 1115(a) Medicaid Demonstration Waiver in an amount not to exceed $40,000. Effective upon G&C approval through June 30, 2019. 50% Federal, 50% General Funds.
