Authorize to exercise a retroactive renewal option to the sole source agreement with Hope on Haven Hill Inc., Rochester, NH (originally approved by G&C on 8-3-16, late item #A), for the provision of substance use disorder treatment services, residential and/or supportive housing, and wraparound services to pregnant and/or parenting women who are experiencing substance use disorders, have income at or below the 185% Federal Poverty Level, and are homeless or at risk of becoming homeless, by increasing the price by $500,000 from $964,238 to $1,464,238 and by extending the contract completion date from June 30, 2018 to June 30, 2019. 100% Federal Funds.
