Authorize to enter into an agreement with Classic Optical Laboratories Inc., Youngstown, OH, to provide vision care products and related services for eligible children and adults for distribution to Vision Care Providers who are enrolled Medicaid providers, in an amount not to exceed $316,000. Effective July 1, 20018 or upon G&C approval, whichever is later, through June 30, 2020. 50% Federal, 50% General Funds.
