Authorize to excise a renewal option to a contract with North Country Health Consortium, Littleton, NH (originally approved by G&C on 9-21-16, item #14), for the provision of services for building Community Health Worker capacity to improve chronic disease prevention and management, by increasing the price by $80,000 from $280,000 to $360,000 and by extending the completion date from June 30, 2018 to June 30, 2020. 100% Federal Funds.
