Authorize to enter into an amendment to a sole source agreement with Granite Pathways, c/o Fedcap, Concord, NH (originally approved by G&C on 7-13-16, item #6C), to continue Regional Access Point Services that assist individuals with substance use disorders obtain services, by increasing the price by $419,924 from $2,098,587 to $2,518,511 and by changing the completion date from June 30, 2018 to December 31, 2018. 79.16% Federal, 13.55% General, 7.29% Other Funds.
