Description
Authorize to amend an existing sole source agreement with the University of NH, Institute for Health Policy and Practice, Durham, NH (originally approved by G&C on 6-21-17, item #11), to design, develop, implement, maintain and support the operationalization of a population health claims based surveillance system that will support the planning and implementation and evaluation of projects related to chronic disease prevention, detection and management, by increasing the price by $228,825 from $2,311,157 to an amount not to exceed $2,539,982. Effective upon G&C approval through the original completion date of June 30, 2019. 100% Federal Funds.
