Authorize to amend an existing agreement with Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 12-20-17, item #20), to continue assisting Critical Access Hospitals to improve quality of care for Medicare beneficiaries and to implement quality improvement activities over thirteen Critical Access Hospitals, by increasing the price by $373,146 from $81,000 to an amount not to exceed $454,146, and by extending the completion date from August 31, 2018 to August 31, 2021. 100% Federal Funds.
