Authorize to enter into a retroactive agreement with NH Coalition for Citizens with Disabilities Inc., d/b/a Parent Information Center, Concord, NH, for the provision of Family-to-Family Health Information Services that support families as caregivers for children with special health care needs, in an amount not to exceed $357,416.74. Effective July 1, 2017 through June 30, 2019. 30% Federal, 70% General Funds.
