Description

Authorize to enter into a sole source amendment to an existing contract with Evident Change, Madison, WI (originally approved by G&C on 7/10/19, Item #14), to continue providing Structured Decision Making® services utilized and any safety planning or court actions needed to support child safety and reduce child abuse and neglect, by increasing the price limitation by $95,280 from $977,925 to $1,073,205 and by extending the completion date from June 30, 2025 to June 30, 2027. Effective July 1, 2025, upon G&C approval. 100% Federal Funds
