Authorize to amend an existing contract with Public Consulting Group LLC, Boston, MA (originally approved by G&C on 10/4/23, Item #20), for ongoing facilitation, scheduling, and completion of the Supports Intensity Scale-Adult assessment for all individuals receiving, or applying to receive, services through the New Hampshire Developmental Disabilities 1915(c) Medicaid Home and Community-Based waiver, by exercising a contract renewal option by increasing the price limitation by $401,093 from $1,637,117 to $2,038,210 and extending the completion date from June 30, 2025 to June 30, 2027. Effective July 1, 2025, upon G&C approval. 100% Federal Funds
