Authorize to amend an existing contract with Waypoint, Manchester, NH (originally approved by G&C on 6/28/23, Item #56), for continuation of the health care coordination program for Children with Special Healthcare Needs, by exercising a contract renewal option by increasing the total price limitation by $1,189,690 from $1,164,576 to $2,354,266, inclusive of a shared price limitation of $320,000 and by extending the completion dates from June 30, 2025 to June 30, 2027.
