Description
Authorize to amend an existing contract with Gateways Community Services, Inc., d/b/a Gateways Community Services, Nashua, NH (originally approved by G&C on 6/29/22, Item #42), to exercise a contract renewal to continue to manage payments for respite and supplemental support services for caregivers under the New Hampshire Family Caregiver Support Program and the Alzheimer’s Disease and Related Disorders Program, and to update budget sheets to accurately reflect Contractor expenditures, by increasing the price limitation by $746,356 from $3,908,276 to $4,654,632 and by extending the completion date from June 30, 2026 to June 30, 2027. Effective upon G&C approval. 45% Federal Funds, 55% General Funds
