Authorize to amend an existing agreement with the JSI Research and Training Institute Inc., d/b/a Community Health Institute, Bow, NH (originally approved by G&C on 3-25-15, item #10), to continue managing a diabetes prevention project and to hold at least one training event for diabetes educators, by increasing the price by $40,000 from $489,996 to $529,996. Effective upon G&C approval through the original completion date of June 30, 2018. 100% Federal Funds.
