Description
Authorize to enter into retroactive sole source agreements and amend 15 existing contract agreements with the vendors as detailed in letter dated June 15, 2017, to continue to provide access to preventive and reparative dental treatment for individuals participating in the statewide community-based and/or school-based oral health programs, by increasing the price by $519,978 from $2,079,964 to an amount not to exceed $2,599,942, and by extending the completion date from June 30, 2017 to June 30, 2018. Effective July 1, 2017. 57.17% Federal, 42.83% General Funds.
