Authorize to enter into a sole source amendment to an existing contract with Cognosante Consulting LLC, Scottsdale, AZ (originally approved by G&C on 12-1-04, Late Item #E), for continuation of quality assurance services in compliance with the requirements of the Centers for Medicare and Medicaid Services, by increasing the price by $5,120,825 from $24,840,301 to a new amount not to exceed $29,961,126. Effective upon G&C approval through September 30, 2018. 90% Federal, 10% General Funds.
