Authorize to amend a sole source contract with Manchester Community Health Center, Manchester, NH (originally approved by G&C on 6-19-13, item #122), to continue providing community based services to children in NH with neuromotor disabilities/complex health care needs, by increasing the price by $330,000 from $5,279,567.41 to an amount not to exceed $5,609,567.41, and by extending the completion date from June 30, 2017 to March 31, 2018. 30% Federal, 70% General Funds.
