Authorize the Glencliff Home to amend a sole source agreement with Dr. David A. Bogacz, Concord, NH (originally approved by G&C on 4-6-16, item #10), to continue to provide dental services to Glencliff Home residents, by increasing the price by $33,600 from $24,000 to an amount not to exceed $57,600, and by extending the completion date from June 30, 2017 to June 30, 2019. 1% Federal, 20% General, 79% Other Funds.
