Authorize to amend a sole source contract with the Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 12-20-13, item #53), to continue to identify the needs, provide implementation, and evaluation of tasks to assist New Hampshire’s 13 small rural hospitals with improving their telehealth and telemedicine capabilities, and improving quality performance through Lean or other quality improvement training and projects, by increasing the price by $39,151 from $446,940 to an amount not to exceed $486,091. Effective upon G&C approval through August 31, 2017. 100% Federal Funds.
