Authorize to amend an existing agreement with National Jewish Health, Denver, CO (originally approved by G&C on 6-24-15, item #51), for the provision of statewide telephonic tobacco treatment services in order to reduce the health and economic consequences of tobacco use and dependence, by increasing the price by $72,173 in Federal Funds, from $1,424,062 to $1,496,235 with no change to the contract completion date of June 30, 2019. 33% General, 67% Federal Funds.
