Authorize to exercise a retroactive sole source contract amendment with Milton School District (originally approved by G&C on 6-19-13, item #135A), for the provision of Student Assistance Programing to address underage drinking among persons aged 12 to 20, and prescription drug misuse and abuse and illicit opioid misuse and abuse among persons aged 12 to 25, in high need, high risk populations in NH, by increasing the price by $180,000 from $3,239,901.69 to an amount not to exceed $3,419,901.69, and by extending the completion date from September 30, 2016 to June 30, 2018. 75% Federal, 25% General Funds.
