Authorize to enter into an agreement with North Country Health Consortium, Littleton, NH, for Building Community Health Workers Capacity to Improve Chronic Disease Prevention and Management, in an amount not to exceed $280,000. Effective September 1, 2016 or upon G&C approval, whichever is later, through June 30, 2018. 100% Federal Funds.
