Authorize to exercise a renewal option with the vendors as detailed in letter dated August 31, 2016 (originally approved by G&C on 6-18-14, item #110), for the continuation of In Home Care, In Home Health Aide Level of Care, and In Home Nursing Level of Care Services, by increasing the price by $1,603,866.25 from $14,875,233.75 to an amount not to exceed $16,479,100, and by extending the completion date from September 30, 2016 to December 31, 2016. 45% Federal, 55% General Funds.
