Authorize to add Concord Hospital Inc., Concord, NH, to the list of licensed medical providers that was established and originally approved by G&C on 5-6-15, to provide necessary outpatient visits, labs, diagnostic tests and outpatient procedures for clients enrolled in the NH Ryan White CARE Program. No Maximum client or service volume is guaranteed. Accordingly, the, price limitation among all agreements is $100,000 each State Fiscal Year for a total of $200,000. Effective July 1, 2016 or upon G&C approval, whichever is later, through June 30, 2018. 100% Other Funds from the Pharmaceutical Rebates.
