Description

Authorize to amend an existing contract with Classis Optical Laboratories, Inc., Youngstown, OH (originally approved by G&C on 6/29/22, Item #12), which was competitively procured, to provide vision care products and related services for eligible children and adults through enrolled Medicaid Vision Care Providers, by exercising a contract renewal option by increasing the price limitation by $36,000 from $54,000 to $90,000 and extending the completion date from June 30, 2025 to June 30, 2027. Effective July 1, 2025, upon G&C approval. 50.45% Federal Funds, 49.05% General Funds 0.5% Other Funds (as defined in RSA 126-AA:3, I)
