Authorize to enter into a contract with New Hampshire Coalition for Citizens with Disabilities, Inc., d/b/a Parent Information Center, Concord, NH, in the amount of $1,078,000 to operate a statewide Family to Family Health Information Center to provide support to children and youth with special health care needs and their families with information and referral, needs assessments, and coordination of services, as well as education and training to families, providers, and other organizations within the system of care. Effective July 1, 2025, upon G&C approval through June 30, 2027, with the option to renew for up to four additional years. 72.17% Federal Funds, 27.83% General Funds
