Authorize to enter into a sole source amendment to an existing contract with Endur ID, Inc., Hampton, NH (originally approved by G&C on 4/8/20, Item #8), for the continued provision of patient identification bracelets and associated software system services, by increasing the price limitation by $24,715 from $111,833 to $136,548 and by extending the completion date from June 30, 2025 to June 30, 2026. Effective upon G&C approval. 30% General Funds, 70% Other Funds (Provider Fees)
