Authorize to enter into an amendment to a sole source agreement with the University of NH, Institute for Health Policy and Practice, Durham, NH (originally approved by G&C on 6-5-13, item #86), by decreasing the price by $29,556 from $7,072,367 to $7,042,811 as the result of two actions: 1) revising the scope of services for Money Follows the Person, which reduces funding for the work; and 2) developing additional quality and calculation measures for the Long Term Services and Supports populations and making enhancements to the Medicaid Quality Information System and Medicaid Quality website, which increases funding for the work. Effective upon G&C approval through the original end date of June 30, 2017. 100% Federal Funds.
