Authorize to exercise renewal options and amend contracts with the four vendors as detailed in letter dated April 25, 2016 (originally approved by G&C on 11-4-15, item #15; 11-18-15, item #12; 5-6-15, item #17; and 5-27-15, item #32), to provide necessary outpatient visits, labs, diagnostic tests, and outpatient procedures for clients enrolled in the NH Ryan White CARE Program, by increasing the price by $200,000 from $100,000 to $300,000 for Appledore Medical Group Inc., and Frisbie Memorial Hospital, and by $200,000 from $200,000 to $400,000 for the other two vendors, and by extending the completion date from June 30, 2016 to June 30, 2018. No maximum client or service volume is guaranteed. Accordingly, the price limitation among all Vendor Agreements combined is $100,000 per each State Fiscal Year. 100% Other Funds from the Pharmaceutical Rebates.
