Authorize to exercise renewal options and amend contracts with the nine vendors as detailed in letter dated May 9, 2016 (originally approved by G&C on 5-27-15, items #29-31; 6-24-15, item #55; 8-5-15, item #24 & #26; and 9-16-15, item #11), to provide outpatient mental health and/or substance abuse counseling services to clients enrolled in the NH Ryan White CARE Program, by increasing the price by $60,000 from $30,000 to $90,000 for Greater Nashua Council on Alcoholism d/b/a Keystone Hall; Harbor Homes Inc., Joan Scanlon, PhD; Community Council of Nashua, NH, d/b/a Greater Nashua Mental Health Center at Community Council and Pastoral Counseling Services UCC Inc., and by $60,000 from $60,000 to $120,000 for the remaining four vendors, and by extending the completion date from June 30, 2016 to June 30, 2018. No maximum client or service volume is guaranteed. Accordingly, the price limitation among all Vendor Agreements combined is $30,000 per each State Fiscal Year. 100% Other Funds from the Pharmaceutical Rebates.
