Authorize to enter into a sole source contract with Powerback Rehabilitation, LLC, Kennet Square, PA, in the amount of $16,324 to provide physical and occupational therapy services to residents of the Glencliff Home. (2) Further authorize to enter into a sole source license for use of premises agreement with Powerback Rehabilitation, LLC, to receive payment in the amount of $16,324 for the use of space onsite at Glencliff Home. Effective July 1, 2025, upon G&C approval through June 30, 2027, with the option to renew for up to four additional years. 100% Other Funds (Rent)
