Authorize to add Abby S. Levin, Plaistow, NH, to the list of Licensed Mental Health and Alcohol and Other Drug Use Professionals that was established and originally approved by G&C on May 27, 2015, to provide outpatient mental health and/or substance abuse counseling services to clients enrolled in the NH Ryan White CARE Program. No maximum client or service volume is guaranteed. Accordingly, the price limitation among all agreements is $30,000 each State Fiscal Year for a total of $90,000. Effective upon G&C approval through June 30, 2018. 100% Other Funds.
