Authorize, for the benefit of the Department of Health and Human Services, to enter into a sole source contract amendment with Interactive Voice Applications Inc., Dallas, TX (originally approved by G&C on 6-13-07, item #20), for software enhancements and to extend the ongoing support, maintenance, and hosting services provided for the email-based Random Moment Sample service, in the amount of $54,000 from $155,000 to $209,000, and by extending the end date from July 1, 2016 to June 30, 2019. 100% Other (Agency Class 27) Funds: the agency class 27 used by DHHS to reimburse DoIT for this contract is 58% General 42% Federal Funds.
