Description
Authorize a sole source contract renewal option with Stephen F. Caldwell, DDS, MS., Derry, NH (originally approved by G&C on 6-18-14, item #62), to provide orthodontics consultant services, by increasing the price by $137,280 from $104,832 to an amount not to exceed $242,112, and by extending the completion date from June 30, 2016 to June 30, 2018. 50% Federal, 50% General Funds.
