Description

Authorize to amend and renew an existing agreement with National Jewish Health, Denver, CO (originally approved by G&C on 6-24-15, item #51), for the provision of statewide tobacco treatment services in order to reduce the health and economic consequences of tobacco use and dependence, by increasing the price by $739,062 from $685,000 to $1,424,062, and by extending the completion date from June 309, 2017 to June 30, 2019. 35% General, 65% Federal Funds.
