Description

Authorize to exercise a renewal option and amend an existing agreement with the JSI Research and Training Institute Inc., d/b/a Community Health Institute, Bow, NH (originally approved by G&C on 3-25-15, item #10), to promote National Diabetes Prevention Programs and conduct a Diabetes Self-Management Education Program workshop, by increasing the price by $265,998 from $223,998 to $489,996. Effective upon G&C approval through June 30, 2018. 100% Federal Funds.
