Authorize to amend an existing sole source agreement with Center for Life Management, Derry, NH (originally approved by G&C on 8-5-15, item #14), to provide permanent housing services through the Federal Continuum of Care program by increasing the price by $10,322 from $243,745 to $254,067 with no change to the current contract end date of June 30, 2016. 100% Federal Funds.
