Authorize to exercise a renewal option and amend an existing agreement with the JSI Research and Training Institute Inc., d/b/a Community Health Institute, Bow, NH (originally approved by G&C on 3-25-15, item #10), to continue managing a media campaign that targets adults at high risk for type 2 diabetes and conduct a Diabetes Self-management Education Program workshop by increasing the price by $265,998 from $223,998 to $489,996, and by extending the completion date from June 30, 2016 to June 30,2018. 100% Federal Funds.
