Authorize to enter into a sole source amendment with New Futures Inc., Concord, NH (originally approved by G&C on 7-22-15, item #21), by including a State Youth Treatment Planning provision to the existing contract for Family and Community Support Networks for Families of Children with Serious Emotional Disturbances and Substance Use Disorders and by increasing the price by $29,920 from $149,138.89 to an amount not to exceed $179,058.89. Effective upon G&C approval through the original end date of September 30, 2016. 100% Federal Funding.
