Description

Authorize to add Frisbie Memorial Hospital, Rochester, NH to a list of licensed medical providers, with the ability to expand to include additional licensed medical providers to provide necessary outpatient visits, labs, diagnostic tests, and outpatient procedures for clients enrolled in the NH Ryan White CARE Program. No Maximum client or services volume is guaranteed. Accordingly, the price among all agreements is $100,000. Effective upon G&C approval through June 30, 2016. 100% Other Funds.
