Description
Authorize the Bureau of Population Health and Community Services, Oral Health Program, to exercise a renewal option and enter into a retroactive amendment to an agreement with Sullivan County Oral Health Collaborative Inc., Claremont, NH (originally approved by G&C on 7-10-13, item #48), to provide access to preventive and reparative oral health treatment for individuals participating in the statewide community and school based oral health program, by increasing the price by $47,896 from $47,896 to $95,792, and by extending the completion date from June 30, 2015 to June 30, 2017. 44.7% Federal, 55.3% General Funds.
