Authorize to enter into a retroactive amendment and exercise a sole source renewal option to an existing contract with NH Hospital Association, Concord, NH (originally approved by G&C on 9-4-13, item #50), to provide Healthcare System Emergency Preparedness Planning, by increasing the price by $1,563,717 from $1,868,818 to an amount not to exceed $3,432,535, and by extending the completion dated from August 31, 2015 to June 30, 2017. 100% Federal Funds.
