Description

Authorize to exercise a renewal option and amend a contract with the Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 9-3-14, item #20), to continue assisting Critical Access Hospitals to improve quality of care for Medicare beneficiaries, by increasing the price by $260,476 from $279,000 to an amount not to exceed $539,476, and by extending the completion date from August 31, 2015 to August 31, 2017. 100% Federal Funds.
