Authorize the Bureau of Behavioral Health to enter into a sole source amendment with Trustees of Dartmouth College, Hanover, NH (originally approved by G&C on 2-8-12, item #30), to complete the project management and evaluation services in support of the NH Healthy Choices Healthy Changes Program to improve the cardiac and metabolic health of individuals with serious mental illness, by increasing the price by $1,361,667 from $3,882,512 to an amount not to exceed $5,244,179, and by extending the completion date from June 30, 2015 to September 30, 2016. 100% Federal Funds.
