Authorize to exercise a renewal option to an existing agreement with vendors as detailed in letter dated June 2, 2015 (originally approved by G&C on 6-19-13, items #131B & #131C), to continue providing pediatric physical therapy services to specialty clinics for children with neuromotor disabilities and their families, by increasing the price by $65,947 from $64,657.40 to an amount not to exceed $130,604.40, and by extending the completion date from June 30, 2015 to June 30, 2017. 30% Federal, 70% General Funds.
