Description
Authorize to exercise a renewal option to an existing agreement with Concord Regional Visiting Nurse Association, Concord, NH (originally approved by G&C on 6-19-13, item #125), for a Registered Nurse to provide community-based care coordination services to children with special health care needs and their families residing in Merrimack County, by increasing the price by $126,078 from $123,612.04 to an amount not to exceed $249,690.04, and by extending the completion date from June 30, 2015 to June 30, 2017. 30% Federal, 70% General Funds.
