Description
Authorize the Bureau of Development Services to exercise a renewal option to an existing agreement with Dr. Cynthia A. Kahn, MD, Developmental Pediatrician, Londonderry, NH (originally approved by G&C on 6-19-13, item #131D), to continue providing developmental pediatric services to specialty clinics for children with neuromotor disabilities and their families, by increasing the price by $229,198 from $224,778.80 to an amount not to exceed $453,976.80, and by extending the completion date from June 30, 2015 to June 30, 2017. 30% Federal, 70% General Funds.
