Description
Authorize to establish a list of dentists, with the ability to expand said list to include additional interested dental contractors, to provide necessary dental treatments for clients enrolled in the NH Ryan White CARE Program. No maximum client or service volume is guaranteed. Accordingly, the price limitation among all agreements is $100,000 each SFY for a total of $200,000. Effective upon G&C approval through June 30, 2016. 100% Other Funds.
