Description
Authorize the Bureau of Development Services to exercise a renewal option to an existing agreement with Manchester Community Health Center, Manchester, NH, (originally approved by G&C on 6-19-13, item #122), to continue providing community based services to children in NH with Neuromotor disabilities, epilepsy, and other special health care needs, by increasing the price by $2,605,985 from $2,673,582.41 to an amount not to exceed $5,279,567.41 and by changing the completion date from June 30, 2015 to June 30, 2017. 30% Federal, 70% General Funds.
