Authorize the Bureau of Population Health and Community Services, Oral Health Program to exercise a renewal option and enter into amendments to existing agreements (originally approved by G&C on 6-19-13, item #93), with the vendors as detailed in letter dated march 13, 2015, to provide access to preventive and reparative oral health treatment for individuals participating in the statewide community and school based oral health program, by increasing the price by $771,894 from $771,894 to $1,543,788, and by extending the completion date from June 30, 2015 to June 30, 2017. 44.7% Federal, 55.3% General Funds.
